Health Coach (Medical Assistant)

We are seeking a full-time, bilingual, and remote Medical Assistant to join our rapidly growing team as a Health Coach. You will be responsible for telephonically delivering high quality care, coaching, and appointment scheduling to assigned patients.
Responsibilities:
• Remotely provide patient health coaching and care to improve patient outcomes
• Coach and educate patients on improving their Chronic Conditions, preventive care and physician directives
• Update and manage patient care plans based on assessment of patients’ needs and physician directives
• Uphold scheduling flexibility between the hours of 8:00am – 7:00pm (PST) to call patients who live in different time zones
Requirements:
• Medical Assistant Certificate
• 2 years+ Medical Assistant experience
• Bilingual (Fluent in English and Spanish)
• Experience working with EHRs/EMRs
• Strong knowledge and background of chronic conditions
• Ability to build rapport with patients
• Compassionate and caring demeanor
• Excellent documentation and verbal communication skills
• Intermediate to advanced computer skills with the ability to navigate multiple systems
• Reliable internet connection
