Must Haves:

• Current and unrestricted RN license in South Carolina
• BLS certification required.
• Minimum 1 year of nursing experience, preferably in an outpatient or primary care setting.
• Strong clinical judgment, communication, and organizational skills.
• Proficiency with EPIC.

Day-to-Day:

We are seeking a compassionate and skilled Registered Nurse to join our ambulatory primary care team. The RN will play a vital role in delivering high-quality, patient-centered care in a fast-paced outpatient clinic setting. This position involves direct patient care, care coordination, health education, and collaboration with a multidisciplinary team to support patients across the lifespan.

Key Responsibilities:

• Perform comprehensive nursing assessments and document findings accurately.
• Administer medications, immunizations, and treatments as prescribed.
• Assist with minor procedures and provide post-procedure care and education.
• Triage patients in-person and via telephone, prioritizing care based on clinical urgency.
• Collaborate with providers and care teams to develop and implement care plans.
• Educate patients and families on chronic disease management, preventive care, and wellness strategies.
• Monitor and follow up on diagnostic test results and patient progress.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Participate in quality improvement initiatives and ensure compliance with clinic protocols and regulatory standards.
• Support care coordination efforts including referrals, transitions of care, and community resource linkage.

Qualifications:

• Current and unrestricted RN license in South Carolina
• BLS certification required.
• Minimum 1 year of nursing experience, preferably in an outpatient or primary care setting.
• Strong clinical judgment, communication, and organizational skills.
• Proficiency with EPIC.
