What would you do as a Nurse Assessment CoordinatorThe Nurse Assessment Coordinator (NAC) at Butterfield Trail Village plays a pivotal role in the oversight and coordination of the federally mandated Resident Assessment Instrument (RAI) process. This position encompasses managing the Minimum Data Set (MDS) assessments, care area analyses, and the development or revision of comprehensive care plans, all while ensuring compliance with state and federal guidelines. Collaborating seamlessly with an interdisciplinary team-including nursing, therapy, dietary, and social services-the NAC is responsible for attesting to the completion of MDS, which serves as the cornerstone for care plans and directly impacts Quality Measures and reimbursement systems. Acting as the expert resource for the Patient-Driven Payment Model (PDPM), the NAC upholds ethical and timeliness standards, applies advanced nursing clinical skills, and facilitates effective communication across care settings to enhance resident care delivery. This role is essential for maintaining a high-performance, customer-focused culture that embodies the values of excellence and integrity.
Would you be a great Nurse Assessment Coordinator?To excel as a Nurse Assessment Coordinator (NAC) at Butterfield Trail Village, candidates must possess a comprehensive set of skills and qualifications. An active, unencumbered nursing license in Arkansas is essential, with a preference for registered nurses (RNs). MDS certification and a minimum of two years of experience in MDS assessments are crucial for succeeding in this role. Furthermore, candidates should have at least five years of long-term care experience to understand the complexities of resident assessments and care planning.
Proficiency in industry-standard software and tools is required for managing the Resident Assessment Instrument (RAI) process effectively.
