Certified Nursing Assistant - (Per Diem) Position

Job Summary:
The Certified Nursing Assistant (CNA) provides essential personal care services under the supervision of a Registered Nurse. This role involves supporting patients with daily activities and ensuring their comfort and well-being in a home health setting.

Key Responsibilities:

• Travel to patients’ residences to deliver care.
• Ensure safe and high-quality delivery of home health services.
• Educate patients, families, and caregivers about diagnoses and care plans.
• Implement care plans as directed by the case manager.
• Maintain accurate and timely documentation aligned with care plans.
• Assist patients with daily activities such as eating, bathing, and mobility.
• Coordinate with healthcare professionals to ensure comprehensive care.
• Use medical equipment and supplies efficiently and responsibly.
• Perform additional duties as assigned by the supervising Registered Nurse.
• Adhere to the Nursing Practice Act and maintain standards of competent performance.

Qualifications:

• Certification as a Nursing Assistant (CNA).
• Ability to travel to patient locations.
• Strong communication and interpersonal skills.
• Commitment to patient care and safety.
• Experience in home health care is preferred.
