Work schedule: M-F 8am-5pm (1 hour lunch)

The Registered Nurse, Diabetes Educator will organize, coordinate, and provide care management services to patients most at risk for health deterioration, sentinel events, and/or poor outcomes. This role will focus on achieving behavior change goals for patients with/at risk for diabetes and related conditions, which will help them to better clinical outcomes and improved health statuses.

Responsibilities

  • Utilizes the primary care medical home model to deliver care that addresses current diseases and preventative/health maintenance follow-ups
  • Identifies and implements actions for improving population health outcomes
  • Collaborates with care teams to identify appropriate patients to provide diabetes education and comprehensive multidisciplinary services for patients and their families
  • Facilitates behavior changes by conducting individualized patient needs assessments, developing care plans to address needs and clinical health statuses, and involving patients and their families in care plan designs
  • Monitors patient progress according to care plan objectives and provides ongoing feedback and updates to their primary care provider
  • Coordinates information and guidance given to the patient and/or family for improved self-management skills and enhanced patient-provider communication
  • Participates as a member of the multi-disciplinary Care Team towards the development and implementation of services
  • Attends and participates in required meetings, trainings, and exercises; shares pertinent information with staff
  • Share accountability for overall health outcomes for patients by working in coordination with care teams

Teaching & Education

  • Educates and monitors other health professionals, care coordinators, and other personnel working with diabetes patients, their families, and caregivers
  • Educates patients during medical appointments to assist in understanding their disease, medication compliance, specialty services, and/or other medical services recommended by their primary care provider, as needed

Record-Keeping

  • Completes progress notes in patient medical records and other necessary paperwork for care coordination and other services or education provided
  • Initiates and processes Unusual Occurrences per documentation guidelines
  • Prioritizes activities according to need and required follow-ups
  • Provides accurate and timely reports to supervisor and site staff, as directed

Qualifications

Education/Experience

  • HS Diploma/GED required; Associate degree in nursing required
  • Bachelor's degree in nursing preferred
  • CA RN license required
  • Active AHA BLS certification required upon hire
  • CDCES certification is required, or individual must obtain the CDES within 2 years of employment
  • Two years of experience providing diabetes and/or health education to patients preferred
  • One year of experience working in a non-profit community health organization or other non-profit service provider preferred
  • Bilingual (English/Spanish) preferred

  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with biological and social sciences, communication, counseling, and education to improve patient self-management
  • Ability to successfully manage multiple tasks simultaneously
  • Ability to analyze, evaluate, and use critical thinking to make decisions and solve problems
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner

Physical Requirements

  • Ability to lift/carry 50 lbs/weight
  • Ability to stand for long periods of time

Neighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans. Pay range: $50.10 - $75.10 per hour, depending on experience. This role is eligible for a $2,500 signing bonus.

While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
