Work schedule: M-F 8am-5pm (1 hour lunch)
The Registered Nurse, Diabetes Educator will organize, coordinate, and provide care management services to patients most at risk for health deterioration, sentinel events, and/or poor outcomes. This role will focus on achieving behavior change goals for patients with/at risk for diabetes and related conditions, which will help them to better clinical outcomes and improved health statuses.

Pay range: $50.10 - $75.10 per hour, depending on experience. This role is eligible for a $2,500 signing bonus.
