Look no further than Saint Luke's!_** **Job Details:** **Full-time; Monday-Friday 7:30am - 4:00pm; In-Person** **Provides social work services to patients in an acute care setting for patients with acute, chronic or terminal illnesses and their families** **Services to patients and their families include: identification of psychosocial needs, psychosocial assessment, discharge planning, crisis intervention, information and referrals, protective services, and patient/family education** **Functions as a member of a multidisciplinary team for patient care.
