Care Coordinator RN (Case Management / Discharge Planning)


Location: Bakersfield, CA

Shift: Days , 8-hour shifts
Guaranteed Hours: 40 hrs/week
Care Setting: Acute – Inpatient
Unit: Care Coordinator (Case Management – Utilization Review)
Charting System: Cerner

 

Requirements
  • License: Active RN license (OK to submit)
  • Experience:
    • 3 years RN experience in an acute hospital setting
    • Must have prior Care Coordination experience in a CommonSpirit / Dignity / CHI hospital (non-negotiable)
  • BLS required
  • BSN not required
  • Scrubs: Business casual with lab coat
  • Call: None
Role Overview

Focus: Discharge Planning (not UR)
Setting: Inpatient
Typical patient populations & duties:

  • Work with all payer types , including uninsured + homeless populations
  • Perform initial assessments , develop discharge plans , coordinate:
    • Nursing homes/SNFs
    • Home health
    • DME
    • Prior authorizations
  • Daily caseload: 15–28 patients on average
  • Requires high attention to detail, strong communication, and excellent customer service

  • Ability to multi-task and work independently
  • Comfortable in a fast-paced environment with shifting priorities
