Collaborates with the healthcare team to implement improvement strategies that guide optimal care to patients. Responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients. Collaborates with patients, families and the care team using education, goal setting and motivational interviewing to promote a healthy lifestyle. Must be organized, self-motivated, self-directed, and autonomous; willing to teach the healthcare team; and seek professional development in areas of expertise. Works with the clinical team, oversees the disease registries and promotes the use of Sanford approved Clinical Practice Guidelines and Protocol Orders.
