Summary
Provide nursing leadership and guidance to nursing teams. Oversight for coordination and management of the services, content, and quality of hospice and palliative care provided to the client and family/caregiver by the Interdisciplinary Group (IDG). Support the growth of the hospice team. Participating in organizational activities that perpetuate the application of the goals that support our mission and values. Assist in maintaining compliance and regulatory measures by performing regular audits and rounding.
Essential Duties & Responsibilities
Provide clinical, administrative, and economic leadership for the interdisciplinary hospice team
Be a part of the collaborative clinical leadership team that ensures the IDG identifies the patient’s and family’s physical, psychosocial, environmental, spiritual, safety and developmental needs.
Actively participate in continuous quality improvement activities (i.e. quality monitoring, chart audits, PIPs, task forces, etc.) and promote a culture of continuous quality improvement throughout the organization with activities not limited to:
Patients plan of care is individualized based on the care needs identified and care is coordinated within the interdisciplinary hospice
Comprehensive initial assessment and admission of patient to hospice care services is performed in a timely fashion
Interdisciplinary, patient and family-centered plan of care is developed, implemented, evaluated and revised as needed throughout the course of hospice care.
Review of electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards.
Provide insight to leadership regarding identified opportunities for improvement.
Evaluate caseloads of IDT members and make revisions as needed (i.e.: use the staffing grid).
Participate in and ensure all aspects of the patient’s pain and symptoms are adequately assessed an initial, individualized care plan is developed in collaboration with the interdisciplinary team.
Attend interdisciplinary group (IDG) meetings, if applicable, to ensure the appropriate coordination of care and optimal interprofessional collaboration for each patient.
Support the work of the interdisciplinary team through providing direct (face-to-face visits/joint visits) and indirect services (i.e. telephonic support and regular contact) as indicated. This includes the use of telehealth technology to assist the IDT in providing services for patients and as a conduit for patient/family interactions.
Support the implementation of care by the IDG to achieve outcomes of patient/family goals for care.
Promote professional competency related to cultural diversity and health equity.
Practice within scope of practice as outlined by licensing body i.e. NC Board of Nursing, Assume primary responsibility and accountability for the coordination of the patient’s hospice care
Models excellence in interprofessional collaborative practice and patient and family-centered care delivery
Demonstrates role modeling during joint clinical visits and the skill of partnering with patients and families to co-create their desired care experience
Assist team members with problem-solving and implementation of complex plans of care
Assess the competencies of clinical team members and ensure care assignments are made based on matching staff skill/competency to patient/family needs.
Collaborates with organizational leadership through involvement in committees, organizational projects and special events to co-create the care experience by meeting the organizational values.

Minimum Qualifications
Education:
Bachelor of Science in nursing or healthcare related field preferred.
Experience:
Three years relevant clinical experience
Two years of clinical leadership experience (preferred)
Case management experience in home-care, medical-surgical. hospice and/or palliative care settings (preferred)
Demonstrated skill/expertise in pain and symptom management
Demonstrated ability to effectively collaborate and facilitate the work of an interdisciplinary group
Demonstrated understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation
Licensures & Certifications:
Valid NC RN license
Valid Driver’s License and proof of automobile insurance
If RN, Current Certified Hospice & Palliative Care Nurse (CHPN) certification (preferred)
Summary of Physical Requirements

Other Duties:
Participate in the design of clinical operations that are in compliance with internal and external standards, (federal, state, local laws/regulations, Medicare Conditions of Participation, defined clinical best practice) policies, and procedures
Disseminate information to and promote interprofessional collaborative proactive within the IDG.
Develop goals and action plans that support the achievement of organizational and team goals and the Mission and Vision of Four Seasons
Recruit, teach, coach and mentor staff to promote the professional development of team members; evaluate staff performance and develop/implement performance improvement plans as needed
Build and maintain key relationships within the community
Effectively collaborate and facilitate the work of a group
Understand of reimbursement, regulatory requirements, and Medicare Conditions of Participation
Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
Participate in department and staff meeting as required
Participate in ongoing education and training offered by the agency and/or required by nursing supervisor as part of individual education plan of RN
Comply with company policies, procedures, guidelines, and Mission Statement
Practice safety and infection control methods
Assist in providing 24 hour on-call services
Oversight of assignments
Perform other but similar duties as assigned
