Key Responsibilities:

• Deliver direct patient care through medication therapy management and clinical interventions
• Collaborate with physicians, nurses, and interdisciplinary teams to ensure safe and effective drug utilization
• Review, verify, and interpret medication orders for accuracy and appropriateness
• Monitor patient responses and adjust therapies based on clinical data and outcomes
• Educate patients and healthcare staff on drug interactions, side effects, and proper usage
• Participate in formulary management, stewardship programs, and quality improvement initiatives
• Maintain compliance with state and federal pharmacy regulations and institutional policies

Qualifications:

• B.S. or Doctor of Pharmacy (Pharm.D.) from an accredited institution
• Active pharmacist license in the state of practice
• Completion of PGY1 residency (PGY2 preferred for specialty roles)
• Board certification (e.g., BCPS, BCACP) strongly preferred
• Proficiency with EHR systems and clinical decision support tools
• Strong communication, analytical, and documentation skills
