Probationary period will not apply to shift changes.\n\n\n\n\t\nWe are offering an exciting opportunity to join our esteemed team at Montefiore Wakefield Campus, a Magnet-designated hospital recognized for its excellence in patient care.\nOur Social Services Department is currently seeking a highly skilled Social Worker to provide exceptional care to patients on a per-diem basis, with a regular schedule every Sunday 2pm-10pm\nPosition Summary:\nThe Social Worker will provide a range of direct and indirect services for patients and families, including the identification and resolution of psychosocial issues. This role involves collaboration with patients, families, and interdisciplinary teams to ensure safe and timely discharge and treatment planning. The Social Worker will uphold the mission of Montefiore and the Social Services Department, as well as the Code of Ethics of the National Association of Social Workers.\nKey Responsibilities:\n •\tScreen patients for social service needs based on diagnosis and social data.\n•\tConduct psychosocial assessments through interviews and interdisciplinary collaboration.\n•\tDevelop and implement discharge/treatment plans in a timely, collaborative manner.\n•\tFunction independently while utilizing supervision for complex discharge planning.\n•\tMaintain professional conduct, confidentiality, and participate in interdisciplinary conferences.\n•\tDemonstrate leadership in team building, conflict resolution, and psychosocial education.\n•\tParticipate in departmental initiatives including quality improvement and continuing education.\n•\tApply knowledge of legal and ethical issues related to end-of-life care and patient rights.\n•\tIdentify and intervene appropriately in cases of abuse, neglect, and violence.\nQualifications:\n •\tMaster's Degree in Social Work from a NYS-registered or equivalent program.\n•\tLMSW or LCSW licensure preferred; otherwise, must have a Memorandum of Understanding with a licensed Social Worker for supervision.\n•\tStrong understanding of the impact of medical, social, and environmental factors on patient care.
